How it works

  2. Generate drafts: AI produces structured draft notes aligned to your templates.
  3. You finalize in the EHR: Your provider or team reviews the draft and moves it into your EHR.
  4. Provider signs: Once finalized in the EHR, the provider signs off.

Best for: Practices that want maximum time back for providers and minimal after-visit clean-up.


How it works

  1. Set your standards: We configure templates, required elements, and what “done” means for your notes.
  2. Draft + QC: AI generates the first draft, then a trained documentation specialist reviews and corrects for quality, consistency, and completeness.
  3. We enter into the EHR: We move the final note into your EHR using the agreed workflow and formatting.
  4. Provider signs: Your provider reviews the final note in the EHR and signs off.

Choose the tier that matches your typical monthly chart volume.

  • Up to 40 notes/month: $329/month
  • 41–80 notes/month: $549/month

Pricing details: You get a predictable monthly baseline. If you exceed 120 notes, overage notes are billed at $6 each. If typical practice volume exceeds 120 note per month, please contact us for a custom quote.

A completed provider note, entered into the EHR and tied to a patient encounter. We confirm what qualifies during onboarding so billing is consistent.


Generally, no. Plans are priced for predictable coverage. If you have seasonal swings, we can scope a plan that flexes.


Overage notes above 120 are billed at $6 per note. If you consistently go over this amount, we'll work with you to establish custom pricing. 


Yes. Documentation coverage is the foundation and you can add operations support and/or live coverage at launch or anytime your needs demand it. 


Yes! We're here to help you and that means adapting as your needs change. To ensure we can scale appropriately on our end, we just ask for a 30 day notice for any plan level changes.


Absolutely! This can vary from direct integration to independent operation alongside your EHR. Share your EHR and workflow details with us and we’ll confirm the best model. 


Yes, English and Spanish support options are available. With our Codex Live plan, we can also offer medical interpretation services. 


  • Month-to-month options are available for Codex Core and Codex Ops plans; however, longer-term agreements are available if you want dedicated staffing and guaranteed pricing. 
  • Annual agreements are required for Codex Live plans. Annual agreements can be terminated with a 90 day notice period. 


  • For U.S. clients, our team members supporting your workflow live and work in the United States—we never outsource labor. 
  • For international clients, we can staff your support using team members in your local country if preferred. We’ve found that local staffing often improves communication, cultural context, and patient expectations, which helps workflows run more smoothly.
